Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Degree of doctor of medicine or an equivalent degree resulting from a course of education in medicine or osteopathic medicine. The degree must have been obtained from one of the schools approved by the Department of Veterans Affairs for the year in which the course of study was completed. Current, full and unrestricted license to practice medicine or surgery in a State, Territory, or Commonwealth of the United States, or in the District of Columbia. Residency Training: Physicians must have completed residency training, approved by the Secretary of Veterans Affairs in an accredited core specialty training program leading to eligibility for board certification. (NOTE: VA physicians involved in academic training programs may be required to be board certified for faculty status.) Approved residencies are: (1) Those approved by the Accreditation Council for Graduate Medical Education (ACGME), b) OR [(2) Those approved by the American Osteopathic Association (AOA),OR (3) Other residencies (non-US residency training programs followed by a minimum of five years of verified practice in the United States), which the local Medical Staff Executive Committee deems to have provided the applicant with appropriate professional training and believes has exposed the physician to an appropriate range of patient care experiences. Residents currently enrolled in ACGME/AOA accredited residency training programs and who would otherwise meet the basic requirements for appointment are eligible to be appointed as "Physician Resident Providers" (PRPs). PRPs must be fully licensed physicians (i.e., not a training license) and may only be appointed on an intermittent or fee-basis. PRPs are not considered independent practitioners and will not be privileged; rather, they are to have a "scope of practice" that allows them to perform certain restricted duties under supervision. Additionally, surgery residents in gap years may also be appointed as PRPs. Proficiency in spoken and written English. Basic Life Support (BLS) certification is required. Preferred Experience: Prior experience providing collaborative psychiatric care that is co-located in a primary care setting with an inter-professional clinical team. Strong knowledge of psychopharmacology and evidence-based psychiatry and psychological practices. Physical Requirements: The Physical Requirements of this position are concerned with the mental and physical ability of the applicant to satisfactorily perform the duties of the proposed assignment and may include requisite laboratory and other screening as required by Federal regulatory agencies. ["Primary Care Mental Health Psychiatrist (PCMHI) is a component of the inter-professional Veteran Aligned Care Team (PACT). The PCMHI role is to provide support for Primary care providers and the rest of the PACT to assure that the mental and behavioral health needs of the primary care population are addressed. Often, this means that PCMHI staff will take the lead in providing mental health care for individuals identified with mental health conditions or in need of assistance making health-related behavioral changes. In PCMHI, Psychiatrists are part of a primary care team and function in a consultative as well as direct treatment role. The Psychiatrist is a key member of the overall mental health team and serves a critical role in the continuum of care. Duties for this position include but are not limited to: Provides ongoing support to PACT teams. While the PCPs often provide routine medication initiation and management, in many instances, it may be preferable for the Psychiatrist to initiate medication and either treat to target or support PCP continuation of medication, either directly or by supervision of care management. Performs direct in-person or virtual assessments and treatment, when indicated. Appropriately communicates primary care staff and attends primary care clinic meetings. Provides chart based consultation. Maintains availability for informal and \"curbside\" consults as needed by PACT members. Ensures ongoing availability for urgent or emergent mental health situations. Provides diagnostic clarification. Attends PACT huddles for informal consultation and education of the team. Offers and provides educational didactics as desired by mental health or PACT team members. Serves as a liaison between PACT providers and other mental health providers working with PACT Veterans who are receiving ongoing services in general and subspecialty MH clinics. Generally the PCMHI Psychiatrist does not provide direct treatment for such Veterans though should be of help during times of crisis. In such instances, the PCMHI Psychiatrist is expected to make every effort to contact and involve the mental health team of record or the designated specialty walk-in clinician. Initiates and manages appropriate medication assisted treatment (MAT) for substance use disorders (SUDs) for Veterans whose care can be managed in PACT with PCMHI support. Participate in same day access and other co-located collaborative duties with other staff as assigned by the service. Collaborate with the client/family to establish physical, mental, cultural and environmental factors that facilitate or impede realistic goals and activities. Conduct and document findings on Veterans seen in the clinical setting. Determine potential needs for therapeutic changes, diagnostic workups or subspecialty consultation. Order and/or interpret appropriate laboratory tests, x-rays, and other relevant diagnostic studies and procedures. Evaluate Veterans and manage mental health diagnoses according to the usual scope of care to include attention to maintenance of stable conditions, preventive health care and screening.
