Social Work Program Coordinator (ACP-GV)- Office Of Rural Health (ORH) funded
Incumbent serves as the facility Advance Care Planning via Group Visits (ACP-GV) Program Coordinator. The ACP-GV Coordinator is responsible for all aspects of the ACP-GV Program within the facility, affiliated Community Based Outpatient Clinics (CBOCs) and community settings. They oversee the daily operation of the program, develop policies and procedures for program operation and prepare reports and statistics for facility, VISN, and national use. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
